Patient Forms

208 Dental is a paperless office, so when you make an appointment with our staff all new patient paperwork will be texted or emailed to you. Paperwork can then be filled out prior to your appointment at a time that is convenient for you. This option works best when all of the paperwork is filled out in one sitting, if you stop and start paperwork the software will make you start back at the beginning.

We also have the option to fill out paperwork in-office on one of our tablets.

If you would like to do your paperwork on physical paper, please ask the front desk for them to be printed out prior to your appointment.

If filling out paperwork in-office, please arrive 10-15 minutes prior to your appointment time.
